What are the Risks of Hyaluronic Acid Fillers?

In general, injection of hyaluronic acid filler is quite safe. Common complications include bruising, swelling, and pain in the injection area. These complications, however, only appear temporarily and resolve spontaneously in about one to two weeks. Other risks include Tyndall phenomenon, local allergic reactions, bacterial infection, vascular occlusion, tissue necrosis, etc. The most serious of these is vascular occlusion.

Our face is full of blood vessels and nerve. Although the distribution of blood vessels is similar for everyone, there are always some differences. Once hyaluronic acid filler enters into the blood vessels, it will block the blood circulation, causing skin inflammation and tissue necrosis. Even if the skin recovers, there will be permanent scars. In more serious cases, if hyaluronic acid filler has been injected into the blood vessels connecting the eyeballs, it may result in blindness. There are such cases of blindness caused by fillers injection in other countries including Europe, America, Taiwan and South Korea.

The Tyndall phenomenon is a physical phenomenon. When the hyaluronic acid filler is injected into the superficial part of the skin, this will cause the skin to appear bluish under sunlight. If Tyndall's phenomenon is not serious, it will subside naturally within a few months. In more serious cases, it can be degraded with injection of hyaluronidase.

In order to reduce the risk of vascular blockage, experienced doctors will withdraw the syringe before injecting hyaluronic acid filler to ensure that the needle is not in the blood vessel. If blood is seen during the withdrawal, the needle needs to be withdrawn immediately and the fillers will be injected in another position. Alternatively, a blunt cannula can be used to reduce the risk of entering blood vessels or damaging other tissues. With proper cleaning and disinfection of the injection area, the chance of infection is very low.
